If it were me, I would start with you, put your dad's full (adopted) name, the adoptive parents and gr. grandparents if he knows them--as far back as he knows. Then I would put mom's lines the same way. If you don't  mind teacher knowing your dad was adopted, put a note on there saying that, if you want to. Good luck.</description>
